Sometimes, trying to find the right form can be tricky. We’ve devised a way to make finding the right form a bit easer.

Complete this form if you want to commence making regular after-tax or before-tax (salary sacrifice) contributions to the Accumulation Plan via payroll deductions.
Use this form if you are an ESSS DB Fund member and you wish to change or cease making contributions to the Fund.
Complete this form if you are employed by DEECD and wish to have your contributions deducted by salary sacrifice and you have set up an ESSSuper Accumulation Plan.
If you are a State super member (New Scheme, Revised Scheme, Transport Scheme or SERB Scheme), you can set up salary sacrifice online, if you have an ESSSuper member online account.
