Sometimes, trying to find the right form can be tricky. We’ve devised a way to make finding the right form a bit easer.
Simply select an option from Step 1 that sounds like what you want to do, then select an option from Step 2 that best describes your goals. From there, we’ll load the forms most relevant to you.

Step 3 - Your results

Complete this form if you want to commence making regular after-tax or before-tax (salary sacrifice) contributions to your Accumulation Plan account via payroll deductions.
Use this form if you are an ESSS DB Fund member and you wish to change or cease making contributions to the Fund.
Each year New Scheme members can change their contribution rate between 1 February to 30 April and 1 August to 31 October. Members can login to Members Online to complete this change online or download the appropriate form here.
